There's a lot of Your Favorite X threads going around but I haven't seen this one yet. And I don't see why not, given that a lot of these fan songs are awesome. I'm not askng about remixes of ZUN's songs I mean original themes like those found in Len'En, Marine Benefit, Subterranean Hatred, Nansei, etc. List by stage, by game, your top 5,any way you like, I'll list by stage.

Phantasmagoria Trues picked out some good stuff in Solitary (St.3), CO2 (Koakuma), Balance of Power (Unseen 3 Standard), and Battle with a Dream (Shinki).  I also like all of the title themes except the last one.

Evanescent Existence had an awesome St. 6 theme, and as much as I have no interest in ever challenging her again, I like Tsurubami's theme.

Stage 5 probably has the best BGM in Reactivate majestical imperial.

Everything in Mystical Power Plant that you would hear after Saraka shows up is above average at worst.  Those last two boss themes and the Extra boss theme, etc.

Nebula Stream is far and away my favorite theme from Riverbed Soul Saver.

Concealed the Conclusion had G Free and Evening Star.  All the other songs can go to saw wave hell.

I really love original Touhou-style songs. I actually have more of these on my computer than arranges. 
I suggest making a NicoNico account if you haven't got one yet. Some of the composers post their works there from time to time.

First original Touhou song that I ever listened to. Mind blown.

- ４面以降のBOSSっぽい曲
Used to listen this song back in 2010. Really like the intense beginning.

- 風吹く地の暁星 - Dazzling Star
Quite a simple song. The style reminds me of STB.

- 旧作風 - Experiment!!
Original PC-98-style song.

- 雨の降る夜に
Supposedly a theme for Daiyousei. Very dramatic.

- シンデレラサイレンス ～ Come Cute Chu.
Beautiful until 1:20, pretty ugh after.

- なんとなく東方っぽいオリジナル曲を作ってみたた
I'd really like to play a stage with this song as BGM...

- 玉石の協奏曲
Stage 5A theme of Treasure Castle Labyrinth. Most of the song is a buildup for a nice part with ZUNpets.

- 水晶華灯怪導
Extra stage theme from Treasure Castle Labyrinth. One of the most energetic Touhou songs I've ever heard.

- 少女達の弾幕浪曼
Last Word Theme from Treasure Castle Labyrinth.

- 天涙流々 - Princess's wish
Opening theme from Heavenly Gathering of Clouds (not a game).

- 本日ハ雨天ナリ
Stage 2 theme from Heavenly Gathering of Clouds. Love the bass.

- 数奇な女神の采配
Track 04 from 東方不調和. Reminds me of the Stage 3 theme from TD.

- 秋風に吹かれて

I can't possibly name favorite themes for all individual stages since I haven't played many fangames, but I do have a couple overall favorites. "Leaf Ponders the Flower, Flower Ponders the Leaf" has been #1 for me for years, though "Mankind Salvation Plan ~ The Greatest Salvation" quickly shot up the ranks after I discovered that song.
