You've found Koishi, but she's in a bad way. Her ankle is sprained, and she looks like she hasn't slept, bathed, or eaten in a long time. Her third eye is also open, and you had suspicions that her mind was in the same state as Reisen's. Eientei's doctor, Eirin, has noted that Koishi's third eye looks like it was forced open.
